Breast enlargement, breast augmentation or augmentation mammoplasty or just a boob job is a surgical procedure in order to increase the size, shape or fullness of the breast. This surgery is more common among women who want larger, well-shaped breasts, or women who have had breast tissue removed (a mastectomy) or women with underdeveloped breasts or asymmetric breasts. Breast implants are used as part of male-to-female gender reassignment, too.
To undergo breast enhancement surgery in Iran, you have to be 18 years old at least, because the breasts should be fully developed, it is better to wait until the early 20s.
The most commonly used breast implants in Iran are filled with silicone or saline, and both have a silicone exterior.
Saline implants contain salt water which is sterile and this type of implant is empty while inserted, then filled with the saline. Silicone implants are made up of silicone gel, already filled before inserting into the body. Women prefer silicone implants because they look more natural compared to saline breast implants.

Breast enlargement is a minimally invasive surgery.
Recovery from breast augmentation typically takes one to two weeks. Your breasts will be swollen and sore for several days after breast augmentation surgery. You will need to limit your activity for several weeks. In general, recovery from breast augmentation surgery is normally 4 to 6 weeks.
Before the procedure
You will have an initial consultation with the surgeon to discuss your options about the shape, size and material of the implant, the incision site (underneath the breast, armpit, or through the areola of the nipples), as well as the position either above or below the muscle.
Before surgery, it is important to avoid certain medications like aspirin and anti-inflammatory medications which can increase bleeding for at least two weeks. Stop smoking before and after your surgery to accelerate the healing process. And buy a sports bra to wear during recovery. It is advisable to buy a bigger size to fit better after the surgery.
During the surgery
The surgeon chooses intravenous sedation or general anesthesia and after sedating you, the surgery will begin.
Based on the decision you and surgeon made with each other the surgeon will make the incision along the areolar edge or the fold under the breast or the armpit. There may be a little risk of visible scars. There is a belly-button approach for the incision which has a higher complication rate.
Inserting and placing the breast implant
Then chosen breast implant is inserted into a pocket under the pectoral muscle or directly behind the breast tissue, over the pectoral muscle. The method of inserting and positioning breast implants depends mostly on the type of implant, desired enlargement, body type, and surgeon’s decision.
Closing the incisions
Incisions are closed with layered sutures in the breast tissue and surgical tape to close the skin. Over time the incision lines will fade.
After the surgery
You need to give your body an optimal amount of time to heal:
- Cover incisions with gauze dressings, as needed, for a week.
- Swelling and mild bruising are normal and will fade away gradually in the coming weeks.
- Wear a soft and supportive post-surgical bra, sports bra or wide, elastic bandage around your breasts to support and positioning breasts during recovery. Do not wear an underwire bra.
- Do not sleep face down or do not get the wounds wet during the first week after surgery.
- Avoid heavy lifting and exercise for 4-6 weeks.
- Most plastic surgeons will allow patients to shower 24 hours after surgery, but they will request that still water be avoided for 2 weeks after surgery.
- You can return to work and daily routine within days of the surgery.

Are scars from a breast augmentation noticeable?
Scars from a breast augmentation are short and generally well-hidden in the crease underneath your breast and can be covered up by a bra or bathing suit top. Longer incisions going up to the nipple will be made if you have a combination of breast procedures, including a breast lift.
How long do the breast implants last?
A lifetime but the manufacturers do not consider them ‘lifetime devices’, meaning that breast implants have the possibility of leaking over time. If a saline implant leaks, the saltwater is harmlessly absorbed by the body. If the implant does not show deflation, it can potentially be left in for a lifetime. A deflated implant is not a serious event or an emergency. Replacement can be performed at a convenient time. Silicone implants are solid so that even if their outer shell ruptures, the silicone inside remains intact and will likely not leak outside of the shell.
